The overall Lowell Health Compass goal is to improve chronic disease management, among individuals and their families, and outcomes related to diabetes and/or depression. Diabetes and depression were selected because of their high prevalence and serious impact on the health and lives of those affected. The PNs will assist patients in improving health outcomes by helping them navigate through five main components of the health care system along the continuum of care:

  1. Chronic disease screening;
  2. Primary health and behavioral health care, diagnosis, and treatment;
  3. Health education;
  4. Coordination of care; and
  5. Chronic disease self-management skills.

Primary communities of focus include those in Lowell with the greatest health disparities and barriers to care: Cambodians, Latinos (primarily from Puerto Rico, Dominican Republic, and Colombia), Brazilians, and Africans from 12-15 different countries.

The overall goal of the Lowell Health Compass Program is to improve chronic disease management among individuals and their families and to improve outcomes related to their diabetes and depression. Key objectives are to:  

  • Design a corps of Community Health Worker (CHW) Patient Navigators(PNs) to enhance staff team building, patient access, screening, and health care navigation;  
  • Provide continuous linkage of communication within LCHC’s care management team ensuring effective integration of CHWs and behavioral health and primary care providers;  
  • Provide access to coordinated medical and behavioral health care, and support services;  
  • Provide patient education to increase patients’ knowledge of their chronic diseases and treatment options; andPromote individual and family chronic-disease self-management skills and outcomes.

The Cambodian Hepatitis B (CHb) Project is a legacy project funded by the Asian Pacific Community Health Organizations (AAPCHO) which aims to decrease acute and chronic Hepatitis B in the Cambodian community by utilizing local media channels to implement prevention strategies designed to improve knowledge of Hepatitis B among Cambodians, and increase the number of patients screened for Hepatitis B.  The project also aims to increase advocacy among local, state, and national policy makers. For information, Please contact Sidney Liang at 978-322-8539.

Hepatitis B and C are among the leading causes of preventable deaths worldwide. One in 10 Asian and Pacific Islander (API) Americans have chronic hepatitis B infection compared with 1 in 1,000 of Caucasian Americans.
