Lowell Health Compass Program Project Director: Sidney L. Liang, MPA The overall Lowell Health Compass goal is to improve chronic disease management, among individuals and their families, and outcomes related to diabetes and/or depression. Diabetes and depression were selected because of their high prevalence and serious impact on the health and lives of those affected. The PNs will assist patients in improving health outcomes by helping them navigate through five main components of the health care system along the continuum of care:
Primary communities of focus include those in Lowell with the greatest health disparities and barriers to care: Cambodians, Latinos (primarily from Puerto Rico, Dominican Republic, and Colombia), Brazilians, and Africans from 12-15 different countries. The overall goal of the Lowell Health Compass Program is to improve chronic disease management among individuals and their families and to improve outcomes related to their diabetes and depression. =================================== The Cambodian Hepatitis B (CHb) Project is a legacy project funded by the Asian Pacific Community Health Organizations (AAPCHO) which aims to decrease acute and chronic Hepatitis B in the Cambodian community by utilizing local media channels to implement prevention strategies designed to improve knowledge of Hepatitis B among Cambodians, and increase the number of patients screened for Hepatitis B.
