Compass is the U.S Department of Health and Human Services Health Resources and Service Administration’s Bureau of Health Profession for the Patient Navigator Outreach and Chronic Disease Prevention Demonstration Program which aims to reduce barriers and improve outcomes related to health disparities in the City of Lowell, Massachusetts for Cambodians, Latinos, Brazilians, Laotians, and African immigrants in the priority areas of diabetes and depression.  This is a one year grant opportunity utilizing 5 Community Health Workers and other local community agencies. Tobacco Cessation Promising Strategies Project is a capacity building project that subcontracts between Lowell Community Health Center and Asian Pacific Partners for Empowerment, Advocacy, and Leadership (APPEAL) to develop, implement, and integrate best practices, document emerging issues and lesson learned regarding the danger of tobacco usage.  For information, Please contact Sidney Liang at 978-322-8539  

The Cambodian Hepatitis B (CHb) Project is a legacy project funded by the Asian Pacific Community Health Organizations (AAPCHO) which aims to decrease acute and chronic Hepatitis B in the Cambodian community by utilizing local media channels to implement prevention strategies designed to improve knowledge of Hepatitis B among Cambodians, and increase the number of patients screened for Hepatitis B.  The project also aims to increase advocacy among local, state, and national policy makers. For information, Please contact Sidney Liang at 978-322-8539 or Barbara Glass 978-322-8526

The Care Coordination Program is a Visiting Nurse Association’s Women’s Health Network and Men’s Health Partnership Program with funding through the Massachusetts Department of Public Health to clinically support services that help reduce barriers to services and increase compliance which support patient navigation, case management, risk reduction education, chronic disease self management, and lifestyle intervention services. Increased Demand for Services (IDS) is a program of Department of Health and Human Services’ Health Resources and Services Administration (HRSA) aims to providing an integrated and coordinated approach to coordinating care for patients by linking them with services within the health center or collaborating community partners to achieve efficient, accessible, appropriate and high quality care.
